How to Submit an IBC Application or Registration Form:
All completed application or registration forms should be sent to ibc@gwm.sc.edu
All applications must be submitted at least 2 weeks before the next scheduled committee meeting.

What Type of Work Must Receive IBC Approval?
All work involving recombinant DNA, materials derived from humans/primates, biosafety level 2 agents, or biological toxins must be approved by the IBC before work is initiated. The appropriate application and/or registration form described below should be completed and submitted to the IBC based on the type of biological materials used in your laboratory.

Recombinant DNA:
Recombinant DNA Application Form
All work involving recombinant DNA must be reviewed and approved by the Institutional Biosafety Committee (IBC) by submitting this form. These experiments must be approved by the IBC before work is initiated.

Human/Primate Materials:
Biosafety Registration Form for Human or Non-Human Primate Materials
This form must be completed for each human/primate material used in the laboratory. For example, work involving human or primate blood, cell lines, body fluids, or unfixed tissue samples. If these materials are used as part of a project involving recombinant DNA, you must complete the Recombinant DNA Application Form, but this additional form is not required.

Biosafety Level 2 (BSL-2) Agents:
Biosafety Registration Form for Biosafety Level 2 Agents
This form must be completed for each biosafety level 2 agent used in the laboratory. For example, work involving BSL-2 bacteria, viruses, fungi, or parasites.
If the BSL-2 agent(s) are used as part of a project involving recombinant DNA, you must complete the Recombinant DNA Application Form, but this additional form is not required.

Biological Toxins:
Biosafety Registration Form for Biological Toxins
This form must be completed for work involving toxins of biological origin. This would include toxins such as Botulinum neurotoxins, Staphylococcal enterotoxins, Ricin, or Tetrodotoxin.
